Why this rule exists
A review culture without enforcement lasts until the first deadline. Policies make the good path the only path: reviewers, linked work items, and a green PR build.
In practice
On main: minimum reviewers, build validation, no self-approval, and reset votes on new commits. Exceptions go through a named bypass group whose use is audited.
